Job summary

Advanced Recovery Systems, LLC seeks a dedicated Integrated Case Manager to guide patients through financial counseling, insurance verification, and discharge planning during treatment. The role emphasizes comprehensive coordination across aftercare, employment support, and resources to ensure continuity of care.

The ICM collaborates with various teams to resolve barriers, ensures accurate Avatar documentation, and supports patients from intake through discharge with a patient-centered approach.

Qualifications

• Bachelor's degree in a health-related field is preferred.
• One to two years of experience in medical, behavioral healthcare or financial fields preferred.
• Familiar with community resources and adept at resolving financial issues.

Responsibilities

• Meet with patients and manage the integrated case management workflow throughout treatment.
• Develop and review a CMDP, addressing financial, discharge, aftercare, and employment needs.
• Educate patients on insurance benefits and patient financial responsibility and payment options.
• Coordinate with multiple departments to resolve financial and continuity-of-care barriers.
• Maintain accurate documentation in Avatar and ensure timely communication.
