Job summary

Daymark Recovery Services in Lexington, NC is seeking a Case Manager to provide care management, person-centered planning and documentation for clients referred to the Davidson County Case Management program through the Davidson County Court System.

Under supervision, you will address social determinants of health, coordinate referrals, monitor progress, and participate in court activities to report on client status and compliance.

Qualifications

• A minimum of an Associate’s Degree in a human services field is required.
• Bachelor’s degree in a human services field is preferred.
• Experience in case management or MH/SU field is preferred.

Responsibilities

• Provides care management services to individuals under court supervision, addressing social determinants of health and facilitating referrals and resources.
• Attends court activities and meetings with court, probation, or parole to provide updates on client status and progress.
• Monitors client progress and ensures compliance with court requirements and standardized referral processes.
• Performs additional duties as assigned by supervisor.
• Demonstrates strong psychosocial and clinical assessment skills with minimal supervision and prioritizes responsibilities.
