You will link school social workers with community resources and assist clients in obtaining necessary referrals while monitoring their progress.

Applicants should have a minimum of a High School Diploma and demonstrate experience in care management. Extensive internal training and benefits, including medical and 401(k), are offered.

Qualifications

• Valid driver's license is required.
• Able to meet NC qualifications of a Tailored Care Management (TCM) Extender or Qualified Professional.
• Demonstrated experience in providing care management services.

Responsibilities

• Provide case management services linking social workers and community resources.
• Assist clients in overcoming barriers to achieve goals.
• Monitor and report on client progress.
