Qualifications

• Bachelor's Degree in Human Services, Social Work, Psychology, Counseling, Sociology, Behavioral Health, or a related field required.
• 1 year of case management experience preferred; 2+ years in behavioral health preferred.
• Strong verbal and written communication skills.
• Knowledge of community resources and entitlement programs.
• Ability to maintain confidentiality and professional boundaries.
• Valid driver's license with acceptable driving record.

Responsibilities

• Conduct comprehensive assessments and identify participant strengths and barriers.
• Develop, implement, and monitor individualized service and treatment plans.
• Assist participants in establishing recovery goals and outcomes.
• Advocate for access to services and resources across systems.
• Coordinate services among providers, families, and community partners.
• Maintain regular participant contact through office and community visits.
• Document services provided and participant progress accurately.
• Ensure compliance with HIPAA and agency policies.
• Support program reporting and quality improvement activities.
