Job summary

Apis Services, Inc. is seeking a dedicated Case Manager to assess participant needs and coordinate individualized service plans within a holistic, values-based framework.

The role advocates for participants and connects them with critical behavioral health and IDD services, promoting recovery and community integration. Responsibilities include team-based planning, mobile direct care, community outreach, and ensuring 24-hour access to Intensive Case Management.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Minimum of 2 years providing direct support in Behavioral Health, Addiction Recovery, or IDD services; 1 year of Case Management experience preferred.
• Valid driver’s license and intermediate computer skills.

Responsibilities

• Collaborate with a multidisciplinary team to develop, review, and execute individualized recovery plans.
• Deliver mobile, community-based support to help participants access resources, increase community integration, and achieve personal recovery goals.
• Establish relationships with community agencies, identify service gaps, and advocate on behalf of participants.
• Complete all required assessments, progress notes, and administrative records accurately and in compliance with agency, funder, and licensing standards.
• Provide coverage and support as needed to maintain required 24-hour access to Intensive Case Management services.
