Job summary

WVUH West Virginia University Hospitals is seeking a Recovery Coach to work with individuals with Substance Use Disorders within WVU Health System settings and in the community. The role focuses on identifying treatment needs, developing and monitoring personalized plans, and supporting patients to remain engaged in treatment.

Responsibilities include outreach, on-site support in treatment settings, collaboration with a treatment team, and linking patients to resources and supports to sustain

Qualifications

• High school diploma or equiv. required.
• PRSS-SUD or CPRS certification required depending on state.
• Background check (state and federal) required.

Responsibilities

• Mentor patients with Substance Use Disorders and link to outpatient treatment.
• Meet with patients and support persons in person or by phone as requested.
• Develop individualized wellness plans and monitor progress.
• Provide information to the treatment team to inform decision making.
• Link patients to resources including housing, education, recovery supports, and health services.
• Act as liaison to patients' supports, families, providers and referral sources.
• Maintain relationships with WVU Health System staff and external agencies to support recovery awareness.
