Job summary

Indiana Treatment Centers is seeking a compassionate Case Manager to join our multidisciplinary team. You will coordinate services, connect clients with community resources, and advocate for long-term recovery in a fast-paced behavioral health setting.

Ideal candidates have a high school diploma or equivalent, experience in case management or direct services, and knowledge of EHR documentation. This role supports housing, employment, transportation, and other recovery resources within Indiana

Responsibilities

• Provide comprehensive case management services for clients with psychosocial needs.
• Coordinate discharge planning and transitions of care to ensure continuity of services.
• Facilitate interdisciplinary case reviews and communicate client progress, barriers, and treatment recommendations.
• Assist clients with accessing housing, employment, transportation, insurance, benefits, community resources, and recovery supports.
• Build collaborative relationships with referral sources, community agencies, probation officers, courts, employers, and other community partners.
• Advocate for clients while promoting accountability, self-sufficiency, and recovery.
• Maintain timely, accurate, and compliant documentation within the electronic health record (EHR).
• Participate in quality improvement initiatives, compliance activities, and multidisciplinary team meetings.
• Support a positive, recovery-focused treatment environment.
• Perform other duties as assigned.
