Zusammenfassung

Strive Recovery is seeking a Case Manager to coordinate care for clients undergoing residential substance use treatment. You will support treatment planning, discharge planning, and linkage to community resources within a multidisciplinary team.

The role requires CADC or related degree and experience in behavioral health. A valid driver’s license and background screening are required. Competitive benefits and supportive work environment are offered.

Aufgaben

• Conduct client needs assessments and identify barriers to recovery and stability.
• Coordinate and assist with discharge planning and aftercare arrangements.
• Connect clients with housing, transportation, employment, education, medical care, psychiatric services, legal assistance, and recovery supports.
• Assist clients with scheduling appointments and completing referrals to outside providers and agencies.
• Collaborate with therapists, medical staff, BH Technicians, and leadership regarding client progress and care coordination.
• Maintain regular communication with referral sources, probation officers, family members, and outside agencies as authorized.
• Monitor and document client progress toward treatment and discharge goals.
• Maintain accurate, timely, and compliant documentation within the electronic health record.
• Assist clients with obtaining identification documents, insurance applications, benefits, and other supportive services as needed.
• Participate in multidisciplinary treatment team meetings and case consultations.
• Educate clients on available resources, recovery supports, and community services.
• Advocate for client needs while promoting accountability and independence.
• Maintain confidentiality and comply with HIPAA, facility policies, and ethical standards.
• Assist with transportation coordination and scheduling as needed.
• Participate in trainings, staff meetings, and professional development activities.
