Job summary

The role requires CADC/LCADC certification or active pursuit, a high school diploma, and a valid driver’s license. Full-time schedule, Monday–Friday, with opportunities for professional growth and supervision.

Qualifications

• CADC or LCADC certification required, pursuing acceptable.
• Must have high school diploma and valid driver’s license.

Responsibilities

• Provide case management for residential clients and families.
• Coordinate with social service agencies; attend case conferences.
• Open/renew Medicaid, welfare, and other benefits; assist with housing.
• Maintain client charts per DMHAS/CARF guidelines.
• Assist with budgeting, shopping and access to services.
