Job summary

First Step House in Salt Lake City is seeking a compassionate Case Manager to support individuals facing serious mental illness and co-occurring conditions towards housing stability and recovery. You’ll work with a multidisciplinary team in a trauma-informed, recovery-focused environment.

The role emphasizes individualized service planning, connection to community resources, and documentation in HIPAA-compliant systems, with a patient-centered approach and ongoing engagement.

Qualifications

• Bachelor's degree in a human services-related field preferred.
• Strong interpersonal and communication skills with problem-solving abilities.
• Ability to document services accurately and professionally.
• Proficiency with Microsoft Office and electronic documentation systems.
• Reliable attendance and punctuality.

Responsibilities

• Provide comprehensive case management and supportive services to residents.
• Develop individualized service plans and review progress regularly.
• Assist with housing stability, documentation, and eligibility for benefits.
• Coordinate access to healthcare, employment, education, and community resources.
• Maintain HIPAA and other regulatory compliance; complete required documentation.
