Qualifications

• Bachelor's degree in a relevant human services field or equivalent education/experience.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong interpersonal, communication, organizational, and time management skills.
• Ability to work independently while collaborating within a multidisciplinary team.
• Basic computer proficiency and experience with electronic health records and related technology.

Responsibilities

• Build meaningful relationships with individuals and families through compassionate, respectful, recovery-focused support.
• Conduct strengths-based assessments and develop personalized service plans to empower goals.
• Coordinate care with behavioral health providers, medical professionals, hospitals, families, and community partners.
• Connect clients with housing, healthcare, employment, transportation, financial assistance, and other essential resources.
• Advocate for individuals with healthcare providers, courts, government agencies, and community organizations.
• Monitor progress, adjust service plans, and document outcomes to support recovery.
