Job summary

The Road Home in Salt Lake City seeks a Housing Case Manager to deliver direct case management to families transitioning from homelessness into housing. You will collaborate with the team, community partners, and housing providers to support diverse needs across Salt Lake County.

Responsibilities include in-home visits, resource referrals, and trauma-informed, housing-focused support. Compliance with background checks is required.

Qualifications

• Bachelors degree or equivalent experience preferred.
• Experience with diverse populations preferred.
• Must pass pre-employment background check and drug screening.

Responsibilities

• Work with families to stabilize housing and community
• Meet families in homes to assess needs and dynamics
• Provide case management to ~25 families across Salt Lake County
• Coordinate referrals and access to community resources
• Advocate with property managers and assist with waitlists
• Maintain records and attend mandatory trainings
