Job summary

South Middlesex Opportunity Council (SMOC) in Framingham, MA seeks a Full-Time Street Outreach Worker to provide street outreach, engage clients, and conduct comprehensive needs assessments as part of Coordinated Entry efforts. The role requires developing service plans and coordinating with housing and health providers to ensure safety and recovery supports.

The position involves regional travel, collaboration with community partners, and maintaining confidential client records.

Qualifications

• Bachelor’s Degree or 3 years of experience in Human Services or related field is required.
• Sensitivity to low-income people from diverse backgrounds.
• Valid license and reliable transportation.
• Assessment, advocacy and case management skills.

Responsibilities

• Provide street outreach and engage new clients referred to the program.
• Attend meetings at SMOC and partner sites as required by contract and participate in CE committee meetings.
• Ensure data is accurately collected and submitted in VESTA.
• Develop, implement, and monitor an Individualized Service Plan for each participant.
• Coordinate service delivery with other providers and maintain client records and monthly updates.
• Travel throughout the Western region of Massachusetts as required.
