Job summary

South Middlesex Opportunity Council, Inc. in Springfield, MA is seeking a Case Manager, Recovery Coach to provide recovery support services to patients with substance use disorder and help them achieve sustained recovery.

The CMRC will maintain ongoing patient support and act as a motivator. This full-time professional role requires driving clients to activities, attending Drug Court, and developing individualized recovery plans while maintaining confidentiality and coordinating with community

Qualifications

• Recovery Coach Academy training required and CARC designation within 12 months.
• Lived recovery experience is required.
• Bilingual Spanish language skills required.
• Strong written and verbal communication skills.
• Must be able to transport clients to appointments.
• Ability to work as part of a team.

Responsibilities

• Provide individual and group support to referred clients and collaborate with staff.
• Attend weekly Drug Court sessions.
• Establish a recovery plan with each individual.
• Provide information and referrals to community resources.
• Maintain client files and complete paperwork.
• Drive clients to recovery activities.
• Engage clients by addressing needs within or beyond scope of work.
• Participate in team meetings and communicate effectively with staff.
• Maintain confidentiality per laws and funder requirements.
• Ensure compliance with SMOC policies and procedures.
• Other duties as assigned.
