Promises Behavioral Health is seeking a dedicated Full-Time Case Manager to join our Worcester team. The role involves planning, coordinating, and delivering individualized client services during residency and aftercare, with psychoeducational and motivational activities in group and individual settings.

Qualifications

• Strong clinical skills and ability to develop individualized service plans.
• Excellent organizational, analytical, and communication abilities.
• Ability to coordinate services with community resources and families.

Responsibilities

• Perform patient screening, intake, and assessment.
• Develop/refine individualized service plans and coordinate referrals.
• Provide regular case management sessions and document reviews.
• Assist patients in developing motivation for recovery and engaging in early recovery activities.
• Maintain patient records and ensure follow-up post-discharge.
