Job summary

Bay Cove Human Services in Boston seeks a Case Manager to join a multidisciplinary team, providing support, counseling, and community referrals to individuals with substance use and co-occurring disorders.

The role emphasizes coordinating services, developing aftercare plans, documenting in the Electronic Health Record, and ensuring smooth patient transitions. This is a non-exempt, Friday-Monday 7am-3pm shift with a focus on compassionate, person-centered care.

Qualifications

• High School diploma or equivalent is required.
• Bachelor's Degree in social work or related field or equivalent experience required.
• Minimum of one year in Behavioral Health or Human Services.
• Knowledge of health care, detoxification, addiction, co-occurring disorders preferred.
• Ability to speak clearly and persuasively in various situations.
• Working knowledge of Windows, Word, and Google Apps.

Responsibilities

• Orient each patient to the unit and explain rights, guidelines, policies and consent procedures.
• Engage patients in information gathering to inform psychosocial assessment and treatment goals.
• Coordinate services with external treatment providers and assist patients to coordinate appointments.
• Develop aftercare plans, referrals, and secure placement for transition to aftercare.
• Provide individual and group education and recovery support during detox/withdrawal and beyond.
• Document all services and patient response in the Electronic Health Record.
• Work with the team to identify changes in wellbeing and adjust the treatment plan accordingly.
• Provide crisis prevention and intervention and escalate as needed.
• Coordinate discharge in collaboration with nursing and aftercare providers.
• Conduct post-discharge follow-up within 30 days.
