Job summary

COMHAR INC. is seeking a dedicated Recovery Counselor/Case Manager to provide therapeutic support to individuals in recovery and coordinate with a multidisciplinary team in Philadelphia, PA.

The role emphasizes person-centered care, accurate documentation, and helping clients engage with community resources to promote independence and well-being. The position requires a BA or related qualification with MH experience, strong communication skills, and a client-focused approach.

Qualifications

• B.A. in human services or related field with relevant mental health experience.
• Alternative qualifications include an associate degree with 4 years mental health direct care.
• High school diploma or GED with 3 years mental health/psych rehab experience accepted.
• CPS certification paired with 2 years mental health/psych rehab experience is acceptable.

Responsibilities

• Support individuals in directing their own recovery process by promoting autonomy, choice, and self-advocacy.
• Teach, coach, and motivate individuals in developing and applying skills that support recovery and daily living.
• Educate individuals on service options including literacy programs, GED prep, employment, volunteering, and social activities.
• Accompany individuals to access community services and resources as needed.
• Encourage use of natural supports and community resources aligned with individuals’ interests.
• Foster wellness, confidence, and self-worth among individuals receiving services.
• Provide warmline cell phone coverage on a rotating schedule for support and guidance.
• Escort individuals to appointments and services including medical visits, housing, and DPW offices.
