The role collaborates with clinical and medical professionals, community providers, and participants in a holistic, team-based approach.

The position requires a Bachelor’s degree in human services or a related field and at least two years of relevant experience, with willingness to travel in the service area and drive

Qualifications

• Bachelor’s degree in human services or a related field.
• At least two years of general work experience providing support to individuals in behavioral health.

Responsibilities

• Team-Based Planning: participate in a multidisciplinary team to provide holistic, values-based services; help develop recovery plans with participants and staff.
• Direct Participant Support: provide community-based services, educate about available resources, and help participants connect with their communities.
• Documentation and Administrative Responsibilities: complete assessments, maintain records per standards, and support program planning.
