Job summary

Resources for Human Development (RHD) is seeking a Case Manager to coordinate services with participants, families, clinicians, and community partners. You will engage in community-based case management, advocacy, and documentation, promoting a strengths-based approach and participant self-determination.

The role emphasizes collaboration within multidisciplinary teams, development of recovery plans, and facilitating access to behavioral health, housing, employment, and education resources.

Qualifications

• Bachelor's degree in a related field as required or preferred.
• One year of Case Management experience recommended.
• Two years supporting individuals in behavioral health, substance use recovery, IDD, or human services.

Responsibilities

• Participate as a member of a multidisciplinary treatment team and assist in recovery plan development.
• Meet with participants in community settings to assess needs and link to services.
• Coordinate behavioral health, healthcare, housing, employment, education, and community resources.
• Advocate for participant access to needed services and document progress.
• Maintain accurate, timely documentation per agency standards.
