Job summary

Apis Services, Inc. is seeking a compassionate Case Manager to support individuals on their path to recovery, independence, and stability.

You will assess needs, plan services, advocate for access to resources, and coordinate community-based supports in collaboration with multidisciplinary teams. The role emphasizes person-centered, strengths-based practices, comprehensive documentation, and crisis intervention as part of a dynamic, community-based program.

Qualifications

  • Bachelor's Degree in Human Services or a related field.
  • Valid driver's license and reliable transportation.
  • Intermediate computer skills and proficiency with electronic documentation systems.
  • Ability to pass federal and state criminal background checks.
  • Minimum of two (2) years of experience working in human services, behavioral health, or a related field.

Responsibilities

  • Assess participant strengths, needs, goals, and barriers to determine appropriate services and resources.
  • Assist in the development, implementation, and review of individualized recovery plans.
  • Collaborate with participants, natural supports, and approved stakeholders to establish meaningful and achievable recovery goals.
  • Monitor participant progress and make recommendations to support ongoing success.
  • Provide community-based case management services focused on recovery, education, and skill development.
  • Connect participants to healthcare, behavioral health, housing, employment, transportation, financial assistance, and other community resources.
  • Support participants in navigating service systems and overcoming barriers to care.
  • Foster community engagement and encourage participation in meaningful activities and relationships.
  • Advocate for participants to ensure access to needed services and supports.
  • Develop and maintain relationships with community agencies and service providers.
  • Identify service gaps and help develop strategies to address unmet needs.
  • Promote participant empowerment, self-advocacy, and independence.
  • Complete assessments, case notes, treatment documentation, and progress reports in a timely and accurate manner.
  • Maintain compliance with organizational policies, funding requirements, licensing standards, and accreditation expectations.
  • Protect participant privacy and confidentiality in accordance with HIPAA regulations.
  • Participate in a rotating on-call schedule to provide crisis intervention and support services.
  • Respond effectively to participant crises and coordinate appropriate interventions and services.
  • Assist participants in accessing emergency resources and support when necessary.
  • Function as an active member of a multidisciplinary team providing holistic, recovery-oriented services.
  • Participate in team meetings, treatment planning discussions, and case reviews.
  • Communicate regularly with internal and external providers to ensure continuity of care.
  • Perform additional duties as assigned to support program goals and participant success.

Position Summary

The Case Manager is responsible for assessing participant needs, identifying appropriate resources, and coordinating individualized services to support successful recovery outcomes. This position plays a critical role in implementing service and treatment plans, advocating for participants, connecting them with community resources, and ensuring they receive the support necessary to achieve their goals.

Working collaboratively with consumers, natural supports, community providers, and multidisciplinary team members, the Case Manager promotes recovery, community integration, wellness, and self‑sufficiency through a person‑centered and strengths‑based approach.

Essential Responsibilities
Participant Assessment & Service Planning
  • Assess participant strengths, needs, goals, and barriers to determine appropriate services and resources.
  • Assist in the development, implementation, and review of individualized recovery plans.
  • Collaborate with participants, natural supports, and approved stakeholders to establish meaningful and achievable recovery goals.
  • Monitor participant progress and make recommendations to support ongoing success.
Community-Based Support & Care Coordination
  • Provide community-based case management services focused on recovery, education, and skill development.
  • Connect participants to healthcare, behavioral health, housing, employment, transportation, financial assistance, and other community resources.
  • Support participants in navigating service systems and overcoming barriers to care.
  • Foster community engagement and encourage participation in meaningful activities and relationships.

Working Conditions

This position combines office-based and community-based work and may require:

  • Extensive community travel (approximately 75% of participant contacts occur in the community)
  • Home visits, community meetings, and provider appointments
  • Exposure to varying weather and environmental conditions
  • Participation in an on-call rotation for crisis intervention services
  • Flexible scheduling to meet participant and program needs

The work environment may include exposure to:

  • Dust, fumes, and airborne particles
  • Extreme temperatures

  • Loud noise levels
  • Mechanical, chemical, electrical, and other environmental hazards
