Job summary

Northern Ohio Recovery Association (NORA) in Cleveland, OH is seeking a Case Manager to join our nonprofit team. This full-time role focuses on acting as the primary client advocate, coordinating services, and supporting recovery goals through collaborative planning with the treatment team.

You will conduct needs assessments, develop service plans, provide individual and group counseling, and help monitor progress while maintaining compliance with state and agency guidelines.

Qualifications

• High school diploma; associates degree preferred.
• CDCA Certification required.
• Two years experience in chemical dependency treatment.
• Basic understanding of the core functions of effective programming.

Responsibilities

• Coordinate and ensure access to services for clients.
• Participate in treatment planning and reviews with the treatment team.
• Conduct needs assessments and prepare service plans.
• Provide individual and group counseling for case management and life skills.
• Develop discharge plans with the treatment team.
• Maintain clients' charts in compliance with regulations.
• Attend training, staff meetings and supervision sessions.
• Monitor residents for warning signs and refer for support.
