Darke County Recovery Services, DBA Recovery and Wellness Centers of Midwest Ohio is seeking a dedicated Case Coordinator to support clients with case management, resource navigation, and collaborative care in a behavioral health setting.

The role requires a bachelor’s degree in social work or psychology, with preferred master’s degree. You will coordinate services, provide crisis support, document progress, and advocate for clients within various agencies.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or a related field; Master’s degree preferred.
• Experience in behavioral health or social work settings is desirable.
• Ability to work with diverse populations and collaborate with teams.

Responsibilities

• Conduct assessments to identify client needs and create service plans.
• Coordinate services across behavioral health, childcare, and workforce development.
• Provide crisis intervention and motivational interviewing as needed.
• Maintain accurate documentation of client interactions and outcomes.
• Collaborate with multidisciplinary teams to ensure holistic care.
