Job summary

True North Recovery is seeking a case manager to provide comprehensive case management services for participants, advocating for access to housing, employment, medical care and community resources. The role includes coordinating referrals, documenting interactions and developing individualized aftercare plans.

The position requires an Associate's degree in human services or a related field, and, if in recovery, ongoing sobriety and participation in a recovery program.

Qualifications

• Associate's degree in human services or related field required.
• If in recovery, one year of continuous sobriety prior to hire and active participation in a recovery support program.
• Must obtain CDC II within 3 years of hire.
• Valid Alaska driver’s license and insurable under TRUE NORTH RECOVERY INC’s automotive insurance, age 21+.
• Continued employment contingent on satisfactory state/federal background check.

Responsibilities

• Maintains relationships with partnering agencies and referral sources.
• Document billable interactions into AKAIMS using DAP notes.
• Coordinate with referral sources, families, community agencies for treatment options.
• Provide motivational interviewing, social and emotional support.
• Create crisis stabilization plans and coordinate peer supports.
• Assist participants in identifying services and referrals (housing, employment, mental health, medical).
• Develop and update personalized plans for housing, employment, support, and treatment.
• Assist participants in exploring available grants to fund housing, transportation, education.
• Attend trainings and staff meetings; participate in service delivery improvements.
• Transport participants on activities and outings as needed.
• Maintain respectful relationships with participants, families, and peers.
