Job summary

On Demand Counseling in Newton Falls, OH is seeking multiple case managers to help individuals with substance use and mental health disorders access housing, benefits, and supportive services. You will connect clients with resources, coordinate referrals, and support discharge planning within a multidisciplinary treatment team.

The role emphasizes advocacy, accurate documentation in the electronic health record, and building relationships with community agencies.

Qualifications

• Must be at least 18 years old.
• Background check (BCI/FBI) and pre-employment drug screen.
• Minimum of a high school diploma or equivalent.
• Minimum of one year of case management experience required.
• CDCA credential preferred; credentialing support available for the right candidate.

Responsibilities

• Assist clients with obtaining and maintaining basic needs, including food assistance, Medicaid, housing, identification documents, transportation, and other community resources.
• Coordinate referrals to medical, behavioral health, vocational, and social service providers.
• Collaborate with counselors and the multidisciplinary treatment team to support client progress.
• Assist with discharge planning and linkage to ongoing community supports.
• Advocate for clients and help them navigate complex systems and services.
• Maintain timely, accurate documentation in the electronic health record.
• Monitor client progress toward case management goals and follow up on identified needs.
• Develop positive working relationships with community agencies and referral partners.
