Freedom Recovery Centers is seeking a Group Facilitator to support clients through recovery by coordinating individualized care plans and guiding them toward achievable goals. You will collaborate with clinical staff and community partners to address housing, employment, medical, and mental health needs in a holistic, client-centered approach.

Competenze

• Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
• CSAC-A or CSAC-S certification required.
• Minimum of 2 years of experience in case management, preferably in substance abuse or behavioral health.

• Conduct initial assessments to determine clients' needs, strengths, and goals.
• Develop and implement individualized care plans with the clinical team.
• Lead groups and coordinate referrals to housing, employment, medical care, and community programs.
