Job summary

Bay Cove Human Services is seeking a Case Manager to join a multidisciplinary team providing support, counseling, and community referrals to individuals with substance use and co-occurring disorders. The role emphasizes coordinating services and documenting care in the patient record.

This is a non-exempt position with a Tuesday–Saturday 11am–7pm schedule, focusing on aftercare planning, discharge coordination, and collaboration with a wide range of partners to support recovery journeys.

Qualifications

• Bachelor's degree in a social work or related field or equivalent experience.
• High School diploma or equivalent required.
• Minimum of one year experience in Behavioral Health or Human Services.
• Familiarity with health care, detoxification, addiction, and DSM terminology preferred.
• Ability to communicate clearly in diverse situations.
• Proficiency with Windows, Word, and Google Apps.

Responsibilities

• Orient new or returning patients to guidelines, rights, and program procedures.
• Engage patients in information gathering to inform psychosocial assessments and goals.
• Coordinate services with external providers and assist attendance of appointments.
• Develop aftercare plans and coordinate transitions to aftercare services.
• Provide individual and group education and recovery support during detox and withdrawal.
