Job summary

Bay Cove Human Services is seeking a Case Manager for Residential Recovery Services in Cambridge, MA. The role involves providing support, counseling, and community referrals to individuals with substance use issues and co-occurring disorders, including individual and group sessions and aftercare planning.

Non-exempt, with a Friday-Monday schedule. The Case Manager works under the Clinical Director, coordinating with external providers and the multidisciplinary team to ensure comprehensive

Qualifications

• High school diploma or equivalent (required).
• Bachelor's degree in social work or related field preferred; equivalent education/experience considered.
• Minimum of one year experience in substance use disorder/behavioral health treatment settings required.
• Certification or eligibility for certification for substance abuse counseling welcome and supported.

Responsibilities

• Orient new admissions to the program and explain rights, guidelines, and procedures.
• Engage residents in information gathering to inform psychosocial assessment and treatment goals.
• Coordinate services with external treatment providers to support residents’ appointments and care.
• Develop aftercare plans and coordinate transitions to aftercare services.
• Provide individual and group education and recovery support during detox/withdrawal.
• Document services in the Electronic Health Record and update progress notes.
• Collaborate with the multidisciplinary team to adjust treatment plans as needed.
• Provide crisis prevention and intervention and escalate when necessary.
• Coordinate discharge planning and post-discharge follow-up within 30 days.
