Job summary

Bay Cove Human Services in Cambridge, MA seeks a Case Manager to provide support, counseling, and community referrals to individuals with substance use and co-occurring disorders, including facilitating individual and group sessions and aftercare planning. This non-exempt, full-time role reports to the Clinical Director and requires collaboration with a multidisciplinary team to coordinate services from admission through discharge.

Schedule: Friday–Monday 7:30am–3:30pm.

Qualifications

• Demonstrated ability to speak clearly and persuasively in a variety of situations.
• Intermediate knowledge of word processing and computer functions (Windows, Word, Google Apps).
• Bachelor's degree preferred; applicable experience considered in lieu of degree.

Responsibilities

• Orient new admissions and inform residents of rights and program policies.
• Coordinate services with external treatment providers and assist with appointments.
• Develop aftercare plans and coordinate transitions to aftercare services.
