Job summary

Bay Cove Human Services in the Greater Boston area is seeking a Case Manager to provide support, counseling, and community referrals for individuals with substance use and co-occurring disorders. The role includes facilitating individual and group counseling sessions and coordinating aftercare planning to support recovery.

The position is non-exempt and involves coordinating with a multidisciplinary team to ensure comprehensive treatment and a warm transition to aftercare services.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in social work or related field preferred; equivalent experience may be considered.
• Minimum of one year experience in substance use disorder/behavioral health treatment settings.

Responsibilities

• Orient residents and explain program rules, rights, and consent procedures.
• Coordinate services with external treatment providers and assist with appointments.
• Develop aftercare plans and coordinate transition to aftercare services.
• Provide individual and group education and recovery support during detox/withdrawal.
• Document all services and resident responses in the Electronic Health Record.
• Collaborate with the multidisciplinary team to monitor resident wellbeing and update treatment plans.
