Job summary

Bay Cove Human Services is seeking a Case Manager to join a multidisciplinary team that provides support, counseling, and community referrals to individuals with substance use and co-occurring disorders. This non-exempt role includes facilitating individual and group counseling and coordinating aftercare planning.

The position operates Tuesday–Saturday, 11am–7pm, and requires documenting all services in the Electronic Health Record while coordinating discharge and post-discharge follow-up.

Qualifications

• Bachelor's degree in social work or related field or equivalent experience.
• High School diploma or equivalent required.
• Minimum 1 year experience in Behavioral Health or Human Services preferred.
• Knowledge of detoxification process, addiction, co-occurring disorders helpful.

Responsibilities

• Orient patients to unit, rights, guidelines, and consent forms.
• Engage patients to inform psychosocial assessment and treatment goals.
• Coordinate services with external treatment providers and attend essential appointments.
• Develop aftercare plans and coordinate transitions to aftercare services.
• Provide individual and group education and recovery support.
• Document services and patient responses in the Electronic Health Record.
• Collaborate with multidisciplinary team and document progress in notes.
• Provide crisis prevention and intervention when needed.
• Coordinate discharge with nursing and aftercare providers and document.
• Post-discharge follow-up within 30 days.
