Qualifications

• High school diploma or equivalent.
• CDCA credential required for consideration.
• QMHS III qualification or ability to meet QMHS III requirements.
• Minimum of one year in behavioral health, SUD treatment, case management, or related field.
• Knowledge of community resources and social-service systems.
• Strong crisis intervention, conflict-resolution and de-escalation skills.
• Valid driver’s license and reliable transportation.
• Ability to manage multiple client needs in a fast-paced setting.

Responsibilities

• Provide case management to clients in substance use disorder and behavioral health programs.
• Assess needs and barriers related to housing, employment, transportation, healthcare, and more.
• Develop and maintain individualized case management/service plans.
• Provide ongoing support and care coordination to meet goals.
• Refer clients to community resources, including housing, employment, mental health, and legal services.
• Coordinate care with therapists, medical providers, and housing staff to ensure continuity.
• Use crisis intervention and de-escalation techniques as needed.
• Maintain professional boundaries and client-centered approach.
• Document timely and accurately per agency policies and regulations.
• Participate in team meetings, supervision, and ongoing professional development.
