You will support access to medical, mental health, social, educational, and vocational services, maintain regular client contact, and connect families with community resources.

Responsibilities

• Provide comprehensive case management and support services to clients participating in substance use disorder treatment programs.
• Advocate for clients navigating healthcare, legal, social service and other systems.
• Help clients access medical, psychological, social, educational and vocational services.
• Assist clients with obtaining public benefits and other community resources.
• Identify unmet needs and help develop appropriate solutions.
• Support clients in achieving goals related to independent living, employment, education, community involvement, and meaningful leisure activities.
