Job summary

Centurion Health seeks a full-time Re-Entry Specialist at Jefferson Correctional Institution in Monticello, FL. You will work with a multidisciplinary team to plan releases, coordinating mental health, medical services and community resources for patients re-entering the community.

The role requires a Bachelor's degree (Master's preferred), TB documentation, CPR certification, and prior release planning experience.

Qualifications

• Bachelor's degree required (Psychology/Social Work/C counseling or related field).
• Master's degree preferred for advanced roles.
• Must maintain tuberculosis documentation and CPR certification.
• Minimum two years' release planning experience preferred.
• Experience in correctional healthcare, criminal justice or forensics preferred.

Responsibilities

• Develop and complete release planning with multidisciplinary team.
• Coordinate behavioral/mental health, medical services and community resources for post-release.
• Collaborate with correctional healthcare programs to ensure smooth transitions.
