Centurion Health seeks a full-time Re-Entry Specialist at Jefferson Correctional Institution in Monticello, FL. You will work with a multidisciplinary team to plan releases, coordinating mental health, medical services and community resources for patients re-entering the community.

The role requires a Bachelor's degree (Master's preferred), TB documentation, CPR certification, and prior release planning experience.

Formación

• Bachelor's degree required (Psychology/Social Work/C counseling or related field).
• Master's degree preferred for advanced roles.
• Must maintain tuberculosis documentation and CPR certification.
• Minimum two years' release planning experience preferred.
• Experience in correctional healthcare, criminal justice or forensics preferred.

We are currently seeking a full-timeRe-Entry Specialistto join our team at Jefferson Correctional Institution located in Monticello, Florida.

The Re-Entry Specialistworks in collaboration with a multidisciplinary team to provide and complete release planning for patients in a correctional setting based on the determination that behavioral/mental health, medical services, and community resources will be required when the patient is released from the correctional environment.

Qualifications: Qualification:

• Bachelor's’ degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master’s degree preferred
• Must maintain current tuberculosis documentation and active CPR certification
• Minimum of two years’ experience in release planning preferred
• Prior experience in working in a correctional healthcare program, criminal justice, or forensics preferred
• Must be able to pass a background investigation and obtain agency security clearance where application
