Centurion Health is seeking a Re-Entry Specialist to join our team at Wakulla Correctional Institution in Crawfordville, Florida. The role focuses on planning post-release care, coordinating with medical and community resources, and ensuring continuity of services for patients leaving custody.

The ideal candidate will hold a Bachelor's degree in a relevant field, with a Master’s preferred, and have experience in release planning, correctional healthcare, or forensics.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Counseling, or related field from an accredited institution.
• Master’s degree preferred.
• Maintain current tuberculosis documentation and active CPR certification.
• Minimum of two years’ experience in release planning.
• Prior experience in correctional healthcare, criminal justice, or forensics preferred.
• Able to pass a background investigation and obtain agency security clearance.

Responsibilities

• Collaborate with a multidisciplinary team to plan releases for patients in the correctional setting.
• Determine required behavioral/medical services and community resources for post-release care.
• Coordinate with facilities to ensure continuity of care upon release.
• Maintain compliance with TB documentation and CPR certification requirements.
