Job summary

Centurion Health is seeking a full-time Re-Entry Specialist at Wakulla Correctional Institution in Crawfordville, FL. You will work with a multidisciplinary team to plan release needs for inmates, ensuring behavioral/mental health, medical services, and community resources are in place for successful reentry.

The role requires a Bachelor's degree (Masters preferred), at least two years' release planning experience, and ability to obtain security clearance.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, or related field; Master's preferred.
• Current tuberculosis documentation and active CPR certification.
• Minimum of two years’ experience in release planning preferred.
• Prior experience in correctional healthcare, criminal justice, or forensics preferred.
• Ability to pass a background investigation and obtain agency security clearance where required.

Responsibilities

• Collaborate with a multidisciplinary team to coordinate release planning.
• Provide and finalize release planning for patients in a correctional setting based on needed behavioral/medical/community resources upon release.
