Job summary
Centurion Health is hiring a full-time Transitional Healthcare Facilitator (Re-Entry Coordinator) for Rockville Correctional Facility in Rockville, Indiana. The incumbent will develop and implement transitional healthcare plans for incarcerated individuals scheduled for release and coordinate with the Indiana Department of Correction and other agencies to ensure pre-discharge planning is completed.

Qualifications
- Bachelor’s degree required.
- Experience with/knowledge of community and state programs and services.
- Two years of experience in re-entry services or release planning preferred.
Responsibilities
- Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release.
- Coordinate with the Indiana Department of Correction and other agencies to ensure pre-discharge planning is accomplished.
- Ensure re-entering individuals are provided with necessary services and resources.
