Reentry Case Manager — Inmate Transition Specialist

Job summary

Leon County in Tallahassee, FL seeks a Reentry Case Manager to perform assessments, develop inmate case plans, and coordinate post-release services. You will work within the Detention Facility to evaluate needs, refer services, and track outcomes, driving program effectiveness and alignment with agency goals.

The role requires strong interpersonal and written communication skills, data collection, and collaboration with internal/external partners.

Qualifications

• High School Diploma or GED is required.
• Bachelor's degree in Social Sciences or related field preferred.
• Experience in criminal justice preferred.
• Strong data tracking and report writing skills.
• Valid driver's license with a satisfactory driving record.
• Strong interpersonal and communication skills.
• Experience with Microsoft Office suite preferred.

Responsibilities

• Perform interviews and assessments to determine offenders' risks and programming needs.
• Prepare case history reports and recommend program assignments.
• Review work and document information to determine progress or changes in assignments.
• Assist the classification unit for caseload needs.
• Counsel offenders on personal problems, employment resources, and transition needs.
• Facilitate re-entry programs and services for offenders.
• Develop, monitor, and amend client transition plans.
• Establish collaborative networks with internal and external partners.
• Communicate with fellow case managers and facility staff to avoid duplicated services.
• Enter data into Apricot as designated by department expectations.
• Conduct research on treatment, transitional, and employment services.
