Centurion Health in Florida is seeking a full-time Re-Entry Specialist at Jefferson Correctional Institution in Monticello to coordinate release planning for inmates in collaboration with a multidisciplinary team. The role focuses on arranging behavioral/mental health, medical services, and community resources needed after release to ensure a smooth transition.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling or related field from an accredited institution
• Master’s degree preferred
• TB documentation current and CPR certification required
• Minimum of two years’ experience in release planning
• Prior experience in correctional healthcare, criminal justice or forensics preferred
• Must pass background investigation and obtain security clearance

Responsibilities

• Provide and complete release planning for patients in a correctional setting
• Collaborate with multidisciplinary team to determine required services post-release
• Coordinate behavioral/mental health, medical services and community resources
• Develop and implement comprehensive release plans
• Ensure readiness and compliance with security requirements
