Centurion Health is seeking a full-time Re-Entry Specialist at Wakulla Correctional Institution in Crawfordville, FL. You will work with a multidisciplinary team to plan release needs for inmates, ensuring behavioral/mental health, medical services, and community resources are in place for successful reentry.

The role requires a Bachelor's degree (Masters preferred), at least two years' release planning experience, and ability to obtain security clearance.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, or related field; Master's preferred.
• Current tuberculosis documentation and active CPR certification.
• Minimum of two years’ experience in release planning preferred.
• Prior experience in correctional healthcare, criminal justice, or forensics preferred.
• Ability to pass a background investigation and obtain agency security clearance where required.

We are currently seeking a full-timeRe-Entry Specialistto join our team at Wakulla Correctional Institution located inCrawfordville, Florida.

The Re-Entry Specialistworks in collaboration with a multidisciplinary team to provide and complete release planning for patients in a correctional setting based on the determination that behavioral/mental health, medical services, and community resources will be required when the patient is released from the correctional environment.

Qualifications:

• Bachelor's’ degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master’s degree preferred
• Must maintain current tuberculosis documentation and active CPR certification
• Minimum of two years’ experience in release planning preferred
• Prior experience in working in a correctional healthcare program, criminal justice, or forensics preferred
• Must be able to pass a background investigation and obtain agency security clearance where application
