Job summary

Proforma Health Inc is seeking a Rapid Response/Code Team Nurse to provide hospital-wide emergency care and guidance, delivering timely, evidence-based interventions.

You will respond to codes (Code Blue and Code White), support and educate staff, assess urgent patient needs, and develop individualized care plans with the care team.

This role emphasizes rapid decision-making, patient safety, and clear communication with patients, families, and colleagues in a high-pressure setting.

Qualifications

• RN license required.
• 2+ years of critical care or ED experience preferred.
• ACLS certification required.
• Associate degree in nursing required; bachelor's degree preferred.
• Ability to work in a safety-sensitive environment.

Responsibilities

• Respond to emergency codes such as Code Blue and Code White.
• Provide clinical support and education to staff for complex procedures.
• Assess and plan patient care in urgent situations.
• Promote patient safety, quality, and regulatory compliance.
• Educate patients and families on illness management, medications, and post-treatment care.
• Develop and communicate individualized care plans with the healthcare team.
• Evaluate physical, psychosocial, and emotional patient needs.
