PATH in Los Angeles, CA is hiring a Case Manager - Outreach to support individuals in ending homelessness. You will conduct intake, develop ISP, link participants to housing, income resources, and health services, and collaborate with partners in a fast‑paced outreach team.

Responsibilities

• Conduct intake and develop Individualized Service Plans (ISP) addressing housing, income, and health needs.
• Provide referrals, linkages, information, and support to resources to achieve ISP goals.
• Complete program assessments (e.g. VI-SPDAT) and 90-day reviews.
• Maintain documentation per PATH policies and ensure accurate participant files.
• Maintain confidentiality per HIPAA and guidelines.
