Job summary

The Salvation Army Beaver Falls area seeks a Housing Case Manager to support households experiencing homelessness by coordinating services, locating housing, and tailoring assistance to help clients achieve permanent housing.

You will conduct intake and assessments, develop and monitor service plans, document contacts in HMIS, and collaborate with community partners while maintaining confidentiality and applying housing-first, trauma-informed approaches.

Qualifications

• BA in human services or related field required.
• Minimum two years in housing, behavioral health, or crisis programs with homeless experience.
• Knowledge of HUD/CoC guidelines and confidentiality expectations.

Responsibilities

• Represent The Salvation Army in a professional, caring manner.
• Provide case management for Rapid Rehousing referrals and related services.
• Coordinate intake/assessment, housing location, lease negotiation, and service planning.
• Link clients to community resources and ensure discharge planning and financial assistance per guidelines.
• Maintain client confidentiality and document contacts in HMIS within deadlines.
• Support clients with transportation and monitor progress toward housing outcomes.
• Collaborate with partners and participate in quality improvement efforts.
