Job summary

The Salvation Army USA Eastern Territory in Buffalo is seeking a Rapid Rehousing/Residential Services Case Manager to join the Buffalo Area Services Team. This full-time role focuses on helping clients rapidly exit homelessness and secure stable housing through a Housing First, person-centered approach.

You'll perform outreach, assess housing needs, connect tenants with landlords, coordinate with service providers, and document progress in HMIS and related systems while maintaining

Qualifications

Responsibilities

• Identify affordable housing options and recruit landlords for RRH participants.
• Educate landlords about RRH and facilitate participation.
• Assess housing needs and determine RRH eligibility.
• Provide direct case management or coordinate with case management.
• Maintain accurate client files and ensure HUD/HMIS documentation.
• Engage tenants with outreach and develop action plans.
• Coordinate eviction prevention and housing permanency planning.
• Connect clients with employment, health, and support services.
