Job summary

Volunteers of America Colorado is hiring a Case Manager for the Back Home SSVF program. You will conduct needs-based assessments, provide case management, connect veterans to benefits, health referrals, and financial resources, and coordinate wrap-around services.

Full time with potential for hybrid work; starting pay is $25.00–$28.00 per hour, based on experience. Colorado driver’s license and a personal vehicle required; agency credentialing within 12 weeks.

Qualifications

• Four years of direct experience in the provision of human services/related experience or education.
• Must possess a Colorado driver’s license and state-mandated automobile insurance.
• Must possess a personal vehicle that may be used for work-related travel.

Responsibilities

• Direct and facilitate delivery of support services as indicated by service modalities.
• Conduct needs-based assessments and engage in community outreach.
• Provide case management and coordinate services across programs.
