Volunteers of America Colorado is seeking a Case Manager for the Back Home SSVF program in Colorado. The role focuses on rapid rehousing for veterans and requires conducting needs-based assessments, coordinating services, and connecting participants with VA benefits and health resources.

A strong emphasis is on housing stabilization and wrap-around support. The position is full-time with potential for hybrid work.

Formación

• Four years of direct experience in the provision of human services/related experience or education.
• Must possess a Colorado driver’s license and state-mandated automobile insurance.
• Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

Responsabilidades

• Direct and facilitate the delivery of appropriate support services for participant households as indicated by relevant service modalities.
• Conduct needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination.
