Job summary

HomeFirst is seeking a Case Manager for the Sonoma Valley RRH program to support clients toward permanent housing. You will manage a caseload, provide housing navigation, and coordinate services with community partners in Sonoma County.

The role emphasizes evidence-based practices, timely documentation, and a commitment to ending homelessness. WFH options are limited to field work within Sonoma Valley.

Qualifications

• 1 year paid or volunteer experience in social work or related human services required.
• BA in Social Work or related field preferred, not required.
• Bilingual in Spanish preferred, not required.
• Familiarity with Housing First, Rapid Re-Housing, and HMIS preferred; willingness to learn Sonoma County RRH standards.

Responsibilities

• Maintain a caseload of 20+ participants and provide comprehensive case management.
• Assist with housing search, landlord engagement, and move-in coordination.
• Document case management activity in HMIS within 5 business days.
• Ensure compliance with contract, agency, and licensing requirements.
• Collaborate with community partners and participate in team meetings.
