Westhab in Yonkers, NY is seeking a Full-time Case Manager for the Rapid Re-Housing (Permanent Supportive Program). You will perform intake, verify eligibility, and coordinate with DSS and housing staff to secure permanent housing for clients.

The role requires a Bachelor or Associate in social work (or related field) and at least two years of direct client services experience; fluent Spanish is highly desirable. Travel within Westchester County is expected.

Competenze

Bachelor or Associate Degree in social work or related field; two years’ direct client services experience preferred.
Fluency in Spanish highly desirable.
Strong written and verbal communication skills; detail oriented and organized.

Conduct client intakes and gather documentation to determine eligibility for referred department/program.
Maintain accurate HMIS progress notes and documentation.
Travel within Westchester County as needed.
Collaborate with DSS and Westhab housing staff to support housing goals.
Coordinate communication among shelters, landlords, and partner agencies.
Identify barriers and develop service plans; refer to mental health and substance abuse services as needed.
Promote housing stability and provide six‑month retention services post-enrollment.
Complete lease documents, program reports, HQS inspections, and rent reasonableness checks on time.
Ensure children's school enrollment and report any child abuse or neglect.
Schedule client home visits and adapt to program needs.
