Job summary

Public Health Management Corporation's Your Way Home Montgomery County program seeks a field-based RRH case manager to support families and individuals experiencing homelessness. The role involves housing assessments, move-in coordination, and trauma-informed, strengths-based practice.

Qualifications

• 3 years of direct service experience in homelessness, housing instability, or related field.
• 1 year of case management experience; housing-focused experience preferred.
• Experience with HMIS or similar case management data systems is preferred.

Responsibilities

• Conduct housing assessments to determine needs, eligibility, and interventions.
• Complete RRH intake assessments, eligibility documentation, leases, move-in inspections.
• Assist participants with housing search meeting HUD Fair Market Rent standards.
• Coordinate move-in logistics including utility setup and essential items.
• Maintain caseload with individualized Housing Stability Plans updated monthly.
• Provide ongoing support and crisis intervention to protect housing stability.
• Liaise with landlords, referral sources, and collaborating agencies.
• Maintain accurate case records and HMIS data per funder requirements.
