HomeFirst is seeking a Case Manager for the Sonoma Valley RRH program. You will locate housing for referred clients, deliver intensive case management, and connect participants with community resources to prevent homelessness and support successful housing outcomes.

The role requires managing a caseload of 12 households (up to 20 participants) and coordinating move-in activities, landlord engagement, and required documentation. Evening/field work may be needed within Sonoma County.

Formación

• 1 year paid or volunteer experience in social work or related human services required.
• Bilingual in Spanish language preferred, not required.
• Knowledge of Housing First, Rapid Re-Housing, Coordinated Entry, and HMIS preferred; willingness to learn Sonoma County CoC RRH Program Standards.
• Experience with housing search, landlord engagement, or housing navigation preferred.

Responsabilidades

• Maintain a caseload of 12 households with up to 20 participants.
• Provide comprehensive case management and housing problem solving.
• Initial eligibility verification, enrollment, ongoing needs assessments, budgeting, information/referral.
• Documentation of case management activities in HMIS within 5 business days.
• Coordinate move-in, unit inspections, leases, and deposits.
